How do I enroll in simpliHŌM’s free healthcare benefit?

You can enroll directly through HŌMhq.

  • Complete the Healthcare Intake Form

    • Log into HŌMhq > agent services > healthcare > healthcare Request Form

    • Complete and submit the Form for yourself and, if desired, your household

  • This form is only for requesting/enrolling in the benefit. It is not where you book appointments.

  • Watch for the confirmation email from simpliHŌM to your simpliHŌM inbox.

    • Email sender: simpliHŌM

    • Subject: Health Care Request Form - [Your Name]

    • Review your info; if anything is incorrect, reply or email support@simplihom.com.

Watch for the Revive healthcare email into your simpliHŌM inbox.

  • Email sender: membercare@info.revive.health

  • Subject: Activate Your New simpliHŌM Benefit

  • Select “Activate My Account.”

  • Save your login and password

  • This creates your Revive account, finalizes enrollment, and unlocks access to your virtual care services. 

Note: You are not fully enrolled and cannot book visits until you activate your account.
