I filled out the healthcare form but haven’t received any emails. What should I do?

 If you submitted the Health Care Request Form and don’t see any emails:

  • Check your simpliHŌM email account.

  • Search for:
    - Health Care Request Form (from simpliHŌM)
    - Activate Your New simpliHŌM Benefit or membercare@info.revive.health (from Revive)


  • Check your spam, junk, and promotions folders.

If you still don’t see either email after checking, email support@simplihom.comLet us know you've completed the Health Care Request Form but haven’t received the confirmation or the activation emails yet.

*Note: office hours are M-F CST. Forms submitted after hours or on weekends will be processed the next business day
