How do I access care once I’m enrolled in the free healthcare benefit?

Once you’ve activated your account through the Revive email, all care is accessed through Revive, not through the simpliHŌM intake form.

To access care:

  1. Open the Revive email titled “Activate Your New simpliHŌM Benefit” and complete activation if you haven’t already.

  2. Log into your Revive Virtual Care Clinic account.
    Use the Revive’s portal/app to:

    • Schedule primary care visits (typically within 1–3 days)

    • Request urgent care (24/7/365, usually within 15–20 minutes)

    • Book mental health therapy sessions (typically within 1–3 days)

    • Access prescription services, including eligible free home delivery

All visits must be scheduled inside Revive after your account is activated.
